• DISTRICT FORMS

    The following forms have been created to facilitate the various processes involved with certification. 

    None of these forms are to be submitted to the Florida Department of Education, these are district created forms for use within the Nassau County School District only.

     

    Notice Form for Additions

    Send to the Human Resources Department to facilitate the addition of a subject or endorsement.  Evidence of the subject area exam must accompany your submission for the addition of a subject.  See Certification Additions for the list of subjects that the district cannot process.

     

    Notice Form for Renewals

    Send to the Human Resources Department to facilitate the renewal of your Professional Teaching Certificate.

     

    Payroll Deduction Authorization Form

    Send the Payroll Deduction Authorization Form to the Human Resources Department, along with the applicable Notice Form above.

     

    District Application for Waiver of General Knowledge

    Send the District Application for Waiver of General Knowledge to the Human Resources Department for processing.  You are not required to provide copies of evaluations if they were earned in the Nassau County School District.  Only employees who earned evaluations in other Florida public schools must provide signed copies of evaluations along with the completed application.

     

    District Application for Waiver of Professional Education Test

    Send the District Application for Waiver of Professional Education Test to the Human Resources Department for processing.  You are not required to provide copies of evaluations if they were earned in the Nassau County School District.  Only employees who earned evaluations in other Florida public schools must provide signed copies of evaluations along with the completed application.
